The Uterine Fibroid Symptom and Quality of Life questionnaire is a validated tool that measures how much your fibroid symptoms are affecting you. It takes about two minutes.
Uterine fibroids can cause heavy menstrual bleeding, pelvic pressure, and disruption to daily life. Answer each question based on your experience over the past three months. Your responses are combined into a symptom severity score from 0 to 100, which places your symptoms in one of three bands: mild, moderate, or severe.
This tool is for information only and is not a diagnosis. It does not replace assessment by a doctor. If you have very heavy or prolonged bleeding, severe pelvic pain, dizziness, or fainting, seek medical care promptly.
